Overview
Medicaid is an income and resource eligible program for New Yorkers who can't afford to pay for medical care. Services covered include inpatient and out patient hospital care, physician services, skilled nursing care, laboratory and X-ray services, optical, dental, early and periodic screening of children, home health care services and medical transportation services. You must use providers who accept Medicaid.
Applying for Medicaid
It is not necessary to appear in person for a Medicaid interview. Persons may call (315) 798-5511 to have an application mailed to them. Applications may be returned by mail with required documentation and an eligibility determination will be made and the individual will be notified of their eligibility. Applications may also be obtained in person at 800 Park Avenue, Utica NY 1st floor or 300 W Dominick St Rome. People under 65 years of age and not in receipt of Medicare can apply through New York State of Health at 1-855-355-5777 or www.NYStateofhealth.ny.gov.

Medical transportation services for persons with active Medicaid coverage may be arranged by calling toll free: 1-855-852-3288. -
Home Care Services
To inquire about home care services available under Medicaid call Office for the Aging and Continuing Care at (315) 798-5456. -
Lost Benefit Cards
To replace a lost Medicaid card, call (315) 798-5511. -
MA/SSI
Supplemental Security Income (SSI) recipients needing Medicaid information should call (315) 793-6270. -
Managed Care
All Family Assistance (FA), Safety Net (SN), and Medicaid (MA) only clients, as well as some Supplemental Security Income (SSI) clients must enroll in Medicaid Managed Care plan unless otherwise expemt. There are no costs, no caps, and no co-payments. Each member has his/her own physician or pediatrician-OB-GYN, who will coordinate all his or her health needs. How do I apply for Medicaid?
Anyone over the age of 65 with Medicare or any age certified disabled (even if you are working) submits an application by mailing to Medicaid Unit, 800 Park Ave, Utica NY 13501 or drop off at the same address.

How do I enroll in a Managed Care Plan?
You must call Maximus at 1-800-505-5678. They will help you pick the best plan for your needs and enroll you in that plan.
My employer offers health insurance, but I can’t afford it. Can Medicaid help?
Yes, If the policy is cost effective, you may qualify for premium assistance. Submit an application (see instructions above) with paystubs and statement from your employer stating the cost of the policy and what is covered, Medicaid will determine if/how much we can help.
I am on Medicare, can I still get Medicaid?
Yes, if you are financially eligible you can have Medicaid as secondary insurance. We may also be able to help with your Medicare premiums.
I have Medicaid and will soon be eligible for Medicare, do I have to enroll in Medicare?
Yes, you must enroll in Medicare. Medicaid will become your secondary insurance and we may be able to help with premium payments.
