  Program Description
  CHBS is a program of Upstate Cerebral Palsy licensed through the Department of Health and the Office of Mental Health.  CHBS provides dual recovery, mental health/psychiatric and primary care treatment through a comprehensive array of services, which include group therapy, individual therapy, medication prescription and management, primary care services, housing and employment services.  Programming is designed to provide dual recovery and mental health services in a comprehensive, individualized fashion that utilizes a strength-based treatment approach.

  Primary diagnosis of mental illness

 

  Treatment Services
  Continuing Day Treatment, Dual Recovery Track, Psychosocial Club, Individual Dual Recovery Counseling, Group Dual Recovery Counseling, Employment Services, Shelter Plus Care services, Primary Care Services (Podiatry, Neurology, Dental, Endocrinology), Psychiatric Services (Psychiatric Evaluations, medication therapy), Mental Health Services (individual, relationship, group and family therapy)

  Discharge Criteria
  Satisfaction of individualized Treatment Plan Goals and Objectives developed by consumer and treatment team.
